What it is
What is musculoskeletal physiotherapy?
Musculoskeletal physiotherapy — often shortened to MSK physiotherapy or MSK physio — is a branch of physiotherapy focused on the muscles, bones, joints, tendons, and ligaments that make up the body’s movement system. An MSK physiotherapist assesses how you move, identifies what’s contributing to your pain or restricted movement, and builds a treatment plan that typically combines:
- A detailed movement and history assessment
- Education about what’s likely happening and why
- A tailored exercise programme built around your specific goals
- Manual therapy where appropriate, as part of a wider plan
- Clear guidance on activity, pacing, and self-management between sessions
MSK physiotherapy is commonly used for back pain, neck pain, shoulder pain, knee pain, hip pain, joint pain, muscle strains, tendon problems, and recovery following injury or surgery. It does not replace urgent medical assessment for red-flag symptoms — see our safety note below.

Why people come to us
When pain stops you doing the things you normally wouldn't think about
Most people who end up looking into physiotherapy for muscle and joint pain aren’t starting from a place of medical curiosity. They’re starting from something specific — a back that’s stopped them lifting their child, a shoulder that won’t let them reach a shelf, a knee that’s made stairs into something to think carefully about, or a hip that’s quietly changed how far they’re willing to walk.
The frustrating part is rarely the pain itself in isolation. It’s the way it starts dictating decisions — which chair to sit in, which side to sleep on, whether today is a “good” day or one to work around. By the time most people search for physiotherapy for back, neck, shoulder, knee, and hip pain, they’ve usually already tried resting it, stretching occasionally, or hoping it settles on its own. Sometimes that works. Often it plateaus because the underlying movement pattern, strength deficit, or loading issue hasn’t actually changed.
What changes that pattern is an accurate assessment — understanding what’s actually driving the problem, not just where it hurts — followed by a plan that’s specific to you rather than a generic stretching handout. That’s the role musculoskeletal physiotherapy plays, and it’s the starting point for every person who comes through Physio Zones doors, regardless of which of the five core pain areas brought them here.
Six core pain areas
The MSK service we deliver across five distinct pain areas.

Back Pain Physiotherapy
From acute lower back pain and sciatica to longer-standing chronic back pain — starting with the movement and loading patterns behind the pain, not just where it hurts.
Neck Pain Physiotherapy
Stiff neck, pain overlapping into the shoulder, or symptoms after a whiplash-type injury — addressed through tailored movement, strengthening and practical workload advice.
Shoulder Pain Physiotherapy
Rotator cuff symptoms, frozen shoulder and general shoulder injury — each needing a different rehabilitation approach, which is why assessment comes before any plan.
Knee Pain Physiotherapy
From knee injury following a sprain or strain to managing knee osteoarthritis with tailored strengthening — built around restoring function, not just chasing pain relief.
Hip Pain Physiotherapy
Hip arthritis, reduced mobility, and groin-area symptoms are assessed carefully, since hip and groin pain can have several different underlying causes.
Joint, tendon & soft tissue
Arthritis, tendonitis, muscle strains, ligament and tendon problems — assessed individually rather than treated with a one-size-fits-all protocol.
Your initial assessment
What to expect from your first appointment
History and context
We start by understanding how the problem developed, what makes it better or worse, and what matters to you — running again, picking up your children without wincing, sleeping through the night.
Movement assessment
A structured movement assessment looking at how the affected area moves and identifying patterns that may be contributing. Clinical reasoning replaces guesswork.
Explanation and plan
A clear explanation of what's likely going on, in plain language, along with a treatment plan that may include a tailored exercise programme and manual therapy where appropriate.
Goals and review
Before you leave, you'll know what the plan involves, roughly how many sessions are anticipated, and when progress will be reviewed — an honest estimate rather than a vague 'we'll see'.

Our approach
Beyond the four pain areas
General joint & muscle pain
If your pain doesn't clearly sit in one of the five core areas, the initial assessment is exactly what it's for. You don't need to self-diagnose before booking.
Arthritis & osteoarthritis
Tailored therapeutic exercise, strength and movement to support function and manage symptoms — particularly for knee and hip joints.
Tendon & soft tissue
Tendonitis and soft tissue injuries — covering muscle, ligament and tendon problems — assessed by tissue, stage of injury and activity demands.
Injury rehabilitation
A structured path back to full function from sprains, strains and soft tissue injuries that don't necessarily need surgical input.

Beyond the clinic
When a clinic appointment isn't the right fit
Not everyone can attend a clinic easily, and not every MSK issue needs
to be addressed in person every time. These options route you to the right pathway.
Home visit physiotherapy
If mobility limitations, recovery from a hospital stay, or general difficulty travelling make clinic visits impractical, we bring MSK assessment and treatment to your home.
Workplace physiotherapy
If your pain is connected to your job — repetitive movements, prolonged sitting, manual handling — a workplace-aware approach often benefits recovery.
Online & Telehealth
For certain presentations, a remote consultation can be a useful first step or a way to maintain progress between in-person sessions.

A note on urgent symptoms
Physiotherapy is well suited to most musculoskeletal pain, but certain symptoms need urgent medical attention. Seek urgent medical help if you experience: sudden loss of bladder or bowel control alongside back pain, numbness around the saddle area, progressive weakness in both legs, unexplained significant weight loss alongside persistent pain, fever with severe back pain, or any sudden, severe symptom that feels different from your usual pattern. This service does not replace urgent medical assessment — contact your GP, NHS 111, or emergency services rather than waiting for a physiotherapy appointment.
Frequently Asked Questions
What is musculoskeletal physiotherapy?
MSK physiotherapy focuses on muscles, bones, joints, tendons and ligaments — the structures responsible for movement. It’s used to assess and treat pain, stiffness, injury, and movement problems, combining assessment, education, tailored exercise, and manual therapy where appropriate.
Do I need a GP referral for private physiotherapy?
In most cases, no. Private MSK physiotherapy generally allows self-referral. If your situation requires medical referral or further investigation, your physiotherapist will tell you clearly and explain why.
How many physiotherapy sessions will I need?
Physiotherapy is commonly part of stroke rehabilitation, and many people find it valuable for functional movement, mobility and personally meaningful goals. We’re careful never to promise a specific recovery timescale — it’s most effective when coordinated with your wider team.
Can physiotherapy support people with Parkinson's disease?
It depends on your condition, how long you’ve had it, its severity, your goals and how you respond to early treatment. Some issues improve within a handful of sessions; others benefit from a longer structured programme. We give you a realistic estimate after assessment.
Can physiotherapy help back pain?
Yes, in most cases. Back pain physiotherapy combines a thorough assessment, movement-based treatment, exercise and practical advice around activity and pacing. Seek medical attention first if you experience any red-flag symptoms.
Can physiotherapy help knee arthritis?
Yes — tailored therapeutic exercise is well-supported as part of managing knee osteoarthritis, alongside strength work and function-focused goals like managing stairs or longer walks more comfortably.
What happens at a physiotherapy assessment?
A typical assessment starts with a discussion of your history, symptoms and goals, followed by a movement assessment. Your physiotherapist then explains their reasoning and proposes a plan — including a realistic sense of how many sessions are likely to be needed.
Do you offer home or online physiotherapy?
Where clinically appropriate and coverage allows, yes. Home visits support people who find clinic visits impractical. Online physiotherapy can support certain MSK presentations as an initial step or between in-person sessions.
Do you offer home or online physiotherapy?
Where clinically appropriate and where coverage allows, yes. Home visit physiotherapy is available for people who find clinic visits impractical due to mobility, recovery from a hospital stay, or other access barriers. Online and telehealth physiotherapy can also support certain MSK presentations, either as an initial step or to maintain progress between in-person sessions. See our Home Visit Physiotherapy and Online & Telehealth Physiotherapy pages for full details on availability.
Ready to get your pain properly assessed?
Whatever pain area brought you here, the right next step is the same: a proper assessment, a clear explanation, and a plan built around your goals — not a generic protocol.
